I have been advised by Network Hearing LLC that the Food and Drug Administration has determined that my best interest would be served if I had a medical evaluation by a licensed physician (preferably by a physician who specializes in diseases of the ear) before purchasing a hearing aid. I do not wish a medical evaluation before purchasing a hearing aid.

By purchasing hearing aids from Network Hearing, you agree that you have been advised by Network Hearing LLC to see a licensed physician prior to purchase a hearing aid, are over the age of 18 years old and do not have any of the medical conditions listed above.

- Acute or Chronic Dizziness
- Pain or Discomfort in the ear
- History of sudden or rapid progressive hearing loss within the previous 90 days
- Unilateral hearing loss of sudden or recent onset within the previous 90 days
- History of active drainage from the ear within the previous 90 days
- Visible congenital or traumatic deformity of the ear
- Visible evidence of cerumen (wax) accumulation or foreign body in the ear canal
- Audiometric Air-Bone Gaps equal to or greater than 15dB at 500 Hz, 1000 Hz and 2000 Hz.
